BLOCKS: Form Block

Form Block

The Form Block allows you to display a Form that you have already created directly on a Dashboard, Portal Page, or Public Page. This makes it easy for users to complete and submit information without needing to navigate away to another area.

  • Replace emails with structured form submissions

What Users Will See

  • The Form displayed clearly within the page
  • Fields that can be completed directly
  • Once submitted, the information is stored based on your Form settings, allowing you to review and manage responses easily.
  • Form Assignments and Automations can be set to trigger on submission of the form.

How to Add This Block

1 Open the Content Block Editor

2 Click the Add Block button

3 Select Form Block from the list.

Once added, the block will be visible on your page and ready for configuration.

How to Configure the Block

After adding the block, you will need to choose which Form to display. Click the arrow to access the dropdown menu inside the block.

All of your active Forms will be available to select from the dropdown and you can click the Form you wish to use and click save.

As soon as a Form is selected, it will automatically appear within the block.  From editing mode you will see a form icon and the name of the form you have selected.

Best Practices

To ensure the best experience for your users, it’s important to keep your Forms simple and easy to follow.

  • Use clear titles and descriptions so users know what the Form is for
  • Keep the number of fields to a minimum where possible
  • Only display Forms to the appropriate audience
  • Test the Form before making it live

Tip: This block works especially well for onboarding workflows, support requests, and any process where you need to collect structured information from users.
