FORMS: Menu of Services (Intent Form)
What is a Menu of Services?
A Menu of Services is what your Targets see when you publish an Intent Form, a Form type whose whole job is to show everything you offer and capture which of it someone wants to move forward with. No payment is collected and no login is required.
Someone opens your link, works through your menu picking the Services that interest them, enters their details, and optionally signs a statement of intent. They arrive in your CRM as a new or matched Contact, and the follow-up attached to each Service they chose runs on its own, before you have even read the lead.
It lives in the Forms area beside your Kickoff, Checkout, Support and Booking Forms, and it is built from the same Items, Packages, Add-ons and Price Bumps you already use elsewhere, so there is nothing to re-enter.
How do I create an Intent Form?
Navigate to Forms and choose Create an Intent Form. The editor opens with the same shape as every other Form, plus two sections that belong to this type alone.

Form Type & Title
Form Type reads INTENT and cannot be changed after creation. Reference Title is required and is how you will find the Form in your own list.
Offer the ability to save this Form as a Draft lets a Target stop part-way and come back rather than losing their selections.
Form Header & Footer
Put your own content, logo or instructions above and below the Form. Each of the Header and Footer has its own content editor, a Hide when embedded switch so the branding does not repeat when the Form sits inside your own website, and its own CSS Class and CSS ID fields.
Intent Options
These two switches decide how firmly you capture a Target's commitment.
Ask for marketing consent adds an unticked switch to the details step. Only Targets who turn it on can be added to a Marketing Audience or sent marketing campaigns, both from this Form and from any Service they select. Leave it off and those particular actions are simply unavailable on your Services.
Require a Signature on submission turns the submission into a signed statement. The Target confirms their selected Services with a signature, and the signed submission is stored as evidence. Leave it off and the same Form collects a plain expression of interest instead.
TIP: An Intent Form sends two emails of its own. The submitter always gets a copy of what they asked for, and an address that already belongs to one of your Contacts also gets a confirmation link. You can rewrite the text of both from the link in this section.
How do I build the menu?
Click Configure Intent Choices to open the menu builder. This is where your Services, their grouping and their limits are set.

Currency
Currency is required, and it sets the currency every price on the menu is shown in. Choose it before you start adding Services.
Items and Packages
Add New Item opens your existing One-Time Items and Packages, each listed with its rate and quantity, and you pick the ones this menu should offer. Nothing is retyped and nothing is duplicated: these are the same records you already maintain in Office > Settings.

Item/Package Section Title is the heading your Target reads above that part of the menu, so you can call it "Our Services", "Choose your package", or whatever suits your business.
Add-ons and Price Bumps
Add-ons and Price Bumps each get their own list and their own section title, built the same way from the records you already have. Use them for the extras that sit alongside a main Service rather than replacing it.
Limiting what someone can pick
Limit # of Items/Packages and Limit # of Add-ons cap how many a Target may select at one time. Leave them empty and the menu is open: a Target can choose as much as they like, and every Service on it is optional.
What else can I ask for?
Below the Intent sections sits Form Blocks, Validation & Order, the same builder every other Form type uses, so an Intent Form is not limited to the menu.
Standard Fields cover the common details, and a new Intent Form starts with Primary Email and First Name already in place. Custom Fields let you collect anything else and store it on the CRM profile. Advanced Blocks add HTML Content Blocks for your own instructions between fields, Choice Blocks to branch the experience on what the Target picks, Appointment Blocks so they can book a time while they are there, and Signature Blocks for additional agreement language.
Each field carries the usual controls: Required, Placeholder, Read Only, Hidden, Subtext, and the Field Styling & Layout Classes that put two or more fields on one row.
What does a visitor experience?
Your Target opens the link with no login. They work through the menu picking the Services they are interested in, watching their selection and running total build as they go, then enter their details and submit, signed or unsigned.
Submitting is the confirmation, so a brand-new visitor is never sent away to an inbox before you hear from them, and no payment is ever collected or requested. If they leave before submitting, nothing is recorded at all.
What happens when someone submits?
What happens next depends on whether SuiteDash already knows the email address. There are three cases, and the difference exists for one reason: nobody should be able to act as one of your existing Contacts just by typing that Contact's email address.
An address you have never seen
A Contact is created immediately with this Form recorded as the lead source, your Contact Intake reactions run, each selected Service's own follow-up runs, and the visitor sees your thank-you.
An address that already belongs to a Contact
The submission is recorded but held. Nothing touches the existing Contact record and no follow-up runs. The person is emailed a single-use confirmation link and sees a "check your email" message. Only when they click that link is the submission released, and the ordinary process then runs exactly as it would have.
Someone already signed in to your Portal
Control of the address is already proven, so there is no hold and no email round-trip. The submission is processed on the spot.
In every case, once the submitter is confirmed the Form's own Contact Intake reactions run first, then the follow-up attached to each Service they actually selected. Nothing fires for the Services they did not choose.
IMPORTANT: The Form's own reactions work on every plan. The follow-up attached to an individual Service is an automation, so it runs on plans that include automations; on a plan that does not, those actions appear disabled with an upgrade prompt rather than failing quietly.
How do I follow up on the interest?
You are notified for each submission, and the notification names which kind it is: a Statement of Intent where a signature was required and given, or an Interest Inquiry where it was not.
On the Contact's Forms tab you can see exactly what they selected and at what prices, which of the two kinds it was, and the lead source, so you can open the conversation already knowing what they want.
How do I deliver the Form?
An Intent Form is published the same way as any other Form. Put it on a Landing Page as your public "Menu of Services" page, place it on a Portal Page for people who are already signed in, embed it in your own website, or simply share the Form's own link.
TIP: Because the link needs no login and collects no payment, it works as a marketing asset in its own right. Put it in an email campaign, on social media, or on your website as the page that answers "what do you actually offer?"
