What is a Quiz Form?

A Quiz Form turns intake into a filter. A prospect takes your branded quiz first, and the right ones land in your CRM with their score already attached. One public link carries the visitor all the way through, from the quiz, to a details form, to a created Contact, with no login required.

A Quiz Form is built around a published quiz, so you will want one ready before you start.

How do I create a Quiz Form?

Navigate to Forms and click +Create Form. In the form-type chooser, select the Quiz type.

Attach a published Quiz

In the Quiz Options panel, select one of your published quizzes to attach. The panel then shows that quiz's read-only settings so you can confirm you picked the right one.

Write the Welcome and Outcome

Enter the Welcome copy the visitor sees before starting, and the Outcome copy they see after finishing. This is your chance to speak to the visitor in your own words, since they never see a raw score.


How do I style and preview a Quiz Form?

Brand the journey with your colors

Open the Colors panel and set your palette. The colors apply across the whole journey, from the welcome screen to the confirmation.

TIP: Custom CSS applies to the details form but deliberately not to the quiz screens, so the quiz stays consistent for every visitor.

Preview the real public page

Use the live preview to walk the real public journey in both desktop and mobile widths. The preview records nothing, so you can step through every screen safely before you share the link.

Share or embed the form

Share the public link, or embed the form on your own website. When embedding, use the recommended minimum height so the taller quiz screens are not cut off.


What does the visitor experience?

The public journey runs in order: Welcome, then the Quiz, then the Outcome, then a Details form, then a Confirmation. It behaves the same when embedded on your own website.

  • No raw score. The visitor only sees the words you wrote, never a number or a pass/fail verdict.
  • Resume where they left off. A visitor who walks away returns to the question they left, not the start.
  • Consistent retakes. Retake behavior follows the policy set when the visitor started.
  • No duplicates. A visitor whose email already exists is matched to that Contact, not duplicated.

What happens if the quiz or form is unavailable?

The form always stays truthful to the visitor:

  • Editing the attached quiz mid-attempt restarts the visitor on the current version, with a calm notice.
  • Unpublishing the quiz blocks new starts, while a visitor already in flight can finish and submit.
  • Deleting the quiz, or deactivating the form, shows a courteous "unavailable" screen rather than an error.
  • If your account loses access to Quizzes, you can still open, edit, and deactivate the Quiz Forms you already own; only creating new ones is blocked.

Where do quiz results appear?

On the Contact profile

Each Contact created or matched by a Quiz Form gets a Quizzes tab that lists every submitted run, newest first, with its score, a pass badge, and a link to review the answers.

On the form's submissions

The Quiz Form's submissions list and detail carry the result alongside the Contact, and each run is addressable by its own link.


How do I reuse or share a Quiz Form?

Duplicating a Quiz Form in the same account keeps everything, including the attached quiz. Saving one as a template, or sharing it through the Community Template Library, keeps all your copy and settings but arrives without the quiz reference, so you re-attach one of your own published quizzes before it can go live.

TIP: Incomplete public quiz attempts that sit idle are automatically cleaned up after 30 days. This never touches submitted results, which are kept.
